To include Form B21 as a case form, do the following:

1. From any window in EZ-Filing, in the main menu bar, select Help>EZ-Filing Help to open the Help window.
2. Select the Search tab.
3. In the Type in the word(s) to search for field, type B21, and click List Topics.
4. In the Select topic list, double-click the topic Include Form B21 (Statement of Social Security Number(s)) as a case form.
5. Follow the guidelines that appear in the right pane of the window.
