Question
You can generate a PDF copy of the EZ-Filing questionnaire and e-mail it to your client, or you can print a paper copy and fax or mail it.
- In the main menu bar, select File>Bankruptcy Forms>Client Questionnaire to open the Client Questionnaire window.
- From the list, select the Debtor Type.
- From the list, select the Nature of Debts.
- In the Extra Sheets area, enter the number of extra sheets you want to include to collect information for assets, liabilities, or executory contracts.
- In the Send to list, do one of the following:
- To save the questionnaire as a PDF file:
- Select PDF File and click OK to open the Save As window.
- Browse to where you want to store the questionnaire file, and click Save.
Tip: Be sure to select a folder that you can find when you attach the file to the e-mail message.
- To print the questionnaire:
- Select Printer and click OK to preview the questionnaire.
- Click

- Fax or mail a printed copy to the client.
- Email a PDF copy to the client.
