How do I electronically file supplemental forms in EZ-Filing version 18.9.5?

Answer

If you need to file an EZ-Filing supplemental form in addition to the case forms, you must file the additional form as a separate action.

In the main menu bar, click File>Bankruptcy Documents>Supplemental Forms.
In the Supplemental Forms window, checkmark Use Electronic Signature, and fill in or revise the Signature Date.
Highlight the form in the list that you want to file, and click Select.

If the form opens in a text editor

Select File>File Electronically from the main menu bar.
In the EZ-ECF® window, use your normal procedure to log in to your court's website and upload the file.

If the form opens a data entry window

Fill in or revise the data, as needed.
To file the selected form, click .
In the EZ-ECF® window, use your normal procedure to log in to your court's website and upload the file.
